Lessons learned from accident responses - issue 257

Issue Number: 257

The goal of emergency response is to prevent and reduce further harm to the maximum extent possible. However, accidents occur where the emergency response seems inadequate and may even commit errors of gross proportions. The following cases were selected as examples demonstrating the variety of difficulties of emergency response. However, one example was picked to show a successful emergency response activity. Emergency plans sometimes fail and these accidents provide some insight into why they may fail. Studying these failures can make a significant contribution to improving emergency response and reducing severity of accident consequences resulting from a poor emergency response.
